The Texas Medicaid Wellness Program is a special health program for people who get Medicaid and have long-lasting or serious health conditions.

People who get traditional fee-for-service Medicaid can join the wellness program.

Most people who can be in this program will be invited to join. A letter will be sent to them. They also will get a phone call from the wellness program.

Nurses to help at any time

Whenever a person in the wellness program or eligible for the program is sick, hurt, or has a health question, they can call a wellness program nurse at 1-800-777-1178 (toll-free). Nurses can help any time of day or night. The service is free. All personal facts are kept private.

Wellness program nurses also can help people in this program with:

• Finding a main doctor.
• Managing their health between doctor visits.
• Learning more about their health conditions.
• Knowing how to take their medicines.
• Picking the best medical care for their health.

Special help

Weight control:
Some people in this program might be able to join the Weight Watchers program at no charge.
