The Texas Health and Human Services Commission (HHSC) announces the issuance of Request for Proposal (RFP) # 529-10-0057 for the Medicaid Health Home Pilot Project. HHSC seeks health home pilot projects in accordance with the specifications contained in this Request for Proposals (“RFP”). The sole point of contact for inquiries concerning this RFP is: The physical address for overnight, commercial mail and hand deliveries is: HHSC Enterprise Contracts and Procurement Services (ECPS) Division All communications relating to this RFP must be directed to the HHSC contact person named above. All other communications between a respondent and HHSC staff concerning this RFP are prohibited. In no instance is a respondent to discuss cost information contained in a proposal with the HHSC point of contact or any other staff prior to proposal evaluation. Failure to comply with this section may result in HHSC’s disqualification of the proposal. |
