• Payment Error Rate Measurement (PERM) Review
  The federal Centers for Medicare and Medicaid Services (CMS) PERM program measures improper payments in the Medicaid and CHIP programs in compliance with the Improper Payments Information Act of 2002.  PERM reviews include tests of Medicaid and CHIP eligibility and claims payments activity during a federal fiscal year.  PERM reviews are conducted once every three years.  [View the Medicaid Error Rate Report for federal fiscal years 2009-2011 (PDF)] The Texas Medicaid error rate is included in fiscal year 2008 error rates.  Texas’ current PERM review will cover claims paid during federal fiscal year 2011 (Oct. 1, 2010 to Sept. 30, 2011).  Providers that have questions about the PERM review process can visit the PERM page of the CMS website, e-mail questions to PERMProviders@cms.hhs.gov or contact A Plus Government Solutions Inc customer service regarding pharmacy/medical record request at 301-987-1100.
