Texas submitted five concept proposals in its state application.

  2. Long-term Care (LTC) Choosing Healthplans All Together (CHAT).
  3. Galveston County 3-share Plan.
  4. 3-Share Access Grants.
  5. Health and Human Services Commission (HHSC) Office of Inspector General. (OIG) Payment Error Rate Methodology ( PERM) Unit.
