Integrated Care Management
Information for Providers


 


Integrated Care Management (ICM) is innovative Texas Medicaid managed care program designed to address the complex care needs of people with disabilities or who are 65 or older. Specialized services, such as health-care coordination and long-term services and supports, will assist participants to perform their day-to-day activities. The ICM model will serve mainly adults who live in the Dallas and Tarrant service areas. [Read the ICM Overview]

The program began enrolling participants in December 2007. Participants began receiving services in February 2008.

HHSC has selected Evercare of Texas, LLC, to be the agency’s implementing partner for ICM. Evercare is affiliated with United Health Group. Medicaid providers in the Tarrant and Dallas Service Areas who are interested in participating in the ICM program must complete a separate contract and credentialing process through Evercare of Texas, LLC.

For information and instructions on contracting with Evercare, providers can call the United Health Care Network Management department at 1-866-574-6088.  

Long-term services and supports providers interested in receiving an Evercare
ICM contract and credentialing application should contact an Evercare
representative at 1-866-915-6474, Option 1, then Option 6.
